Custom Shirts & Hoodies Order Form (READ DESCRIPTION)

Non refundable $5 consult fee will be applied to invoice for credit and deducted from your final balance owed.  Consult fee is non refundable

Please read full description and fill out consult form completely to prevent delays in processing order. 

This form should be filled out for all customizable short sleeve T-shirts, Long sleeve shirts, sweat shirts and Hoodies. Sizes range from infants to adults 

you can add a basic design description, upload your own design or images, including birthday shirts, memorial shirts, business shirts and more 

Don't forget to include placement of design in the description box. (Ex: left side, right side, middle etc).

This is a consult form only, after customization proof has been approved by customer invoice will be send to complete custom purchase. Prices vary depending on customization details.
