Custom Product Order Form

This form should be filled out for all customizable products.

Please allow up to 24 business hours for a initial response to form submitted. Remember orders aren’t place until payment is received. 

If you need a customize product that you don’t see choose the “something else?” And add the product needed in the detail description box. 

Important: for Custom Designs there will be a standard non refundable $15 design consult fee Please see design consult option. 

This is not an official order. 

This is a consult form only, after customization proof has been approved by customer invoice will be send to complete custom purchase. Order will not be processed until full payment is received. 

After initial consult an Invoice for payment will be emailed to the email address provided. Orders will not be processed until invoice has been paid in full.

We have a minimum processing time of  3-5 business days after payment for all custom order. If you need proofs of custom products processing times may be extended. (Larger orders may take longer to process and will be discussed during consult.)

No Refunds on custom products and services.
