Baby Box Bundle Order Form

This is a non refundable $5 consult fee that will go toward your finally Total if items are purchased.

Baby Boxes starts at $35 for a basic personalized name bundle with 3 items. Prices go up from there depending on items added and customization details. (3pc basic bundle example in photos).  

This is a consult form only. After submitted and approval of products and design an invoice will be sent to you to complete purchase and start order.

Order is not placed or processed until full invoice is paid by customer. 

Please fill out all applicable fields, along with choosing each product you would like to be customized. If you dont see a product you need listed choose the “Something else” options and add an description of what is needed. 

If you need an intricate design made from scratch, an additional design fee may be added to order. 

Allow up to 24-48 business hours for inital response for form. Please note that baby box bundles can take up to 5-7 business days for processing plus an additional 4-15 business days for shipping and delivery. So please order in advance.
